Helpful Hints for Taking This Online Survey
How long is the survey?
In most cases, the survey can be completed in 15 to 20 minutes.
Can I start the survey and come back to it?
In order to protect the confidentiality of your answers, the survey cannot be stopped and saved. Be sure to allow enough time to complete it before you begin.
Do I have to take the survey while I am at school?
No, you can take the survey from any computer that has an internet connection – at home, at school or at any other location. To keep your answers confidential, close the browser once you have submitted the survey.
I received an error message when I tried to enter my code. What do I do now?
Please call or email the helpdesk once it is activated
I entered my access code but cannot view the survey. What do I do now?
If you have difficulty viewing the survey after your access code has been accepted, try to access the survey on another computer. A high speed connection will work best, and sometimes the internet options on a computer will not allow you to view the survey. If you try on another computer and continue to have trouble, please call or email the helpdesk once it is activated.

When the survey begins, at the bottom of each page you will see the following buttons:
Back - Takes you back one page within the survey. Remember to use this button instead of the back button on your browser's toolbar to navigate the survey.
Next - Moves you forward one page within the survey. Please note that slight delays may occur as individual computers and browsers may load pages of the survey at different rates.
Submit - This button appears on the last page of the survey. You must submit your responses for them to be included in the database.
Please use these buttons instead of the back and forward buttons on your Internet browser.
